Declaration By Applicant
- 1. I hereby confirm that I am medically fit and normal and free from any diseases.
- 2. I hereby under take to abide by all rules and regulations imposed by INTERNATIONAL SCHOOL OF HOSPITALITY & MANAGEMENT and concerned School(s), College(s), Board(s), University(s), Industry(s), Institution(s) time to time.
- 3. I understand and declare that my admission is provisional and will confirm if eligible by the Director/Authority of the concerned School(s), College(s), Board(s), University(s), Industry(s), Institution (s) and will be cancelled if found not eligible INTERNATIONAL SCHOOL OF HOSPITALITY & MANAGEMENT will not be responsible for that reason.
- 4. I understand and declare the rules and regulation of INTERNATIONAL SCHOOL OF HOSPITALITY & MANAGEMENT and the concerned School(s), College(s), Board(s), University(s), Industry(s), Institution(s) shall also be applicable for the matters those are not covered in the respective prospectus(s) of the School(s), College(s), Board(s), University(s), Industry(s), Institution(s) and I hereby undertake to abide by them.
- 5. I am an applicant where result is awaited, and undertake to produce the Mark sheet(s)/Degree Certificate(s) as a proof for required eligibility criteria before the cut off date, failing which the provisional admission granted will be cancelled.
- 6. I understand the document(s) once submitted will not be refunded under any circumstances.
- 7. I understand the fees once paid shall neither be refunded nor adjusted towards any other payment under any circumstances.
- 8. I understand that the certificate(s)and final degree certificate and other official transcript will be issued by the concerned school(s), College(s), Board(s), University(s), Industry(s), Institution(s) as detailed in the section 3 this application. I also understand that INTERNATIONAL SCHOOL OF HOSPITALITY & MANAGEMENT will not be responsible for any delay and /or any type of misconduct made by the concerned school(s), College(s), Board(s), University(s), Industry(s), Institution (s).
- 9. I hereby declare that particulars furnished above are true to the best of my knowledge and belief. I have read the prospectus and the rules and regulations. I hereby promise to abide by them. I understand that action can be taken against them by the INTERNATIONAL SCHOOL OF HOSPITALITY & MANAGEMENT if I am declared by them to be guilty of any type of misconduct mentioned herein.
- 10. I hereby confirm that I will pay my dues/fees as per schedule dates provided or would be provided to me in future.
You are requested to issue post dated cheques if you are preferring instalments..Any failure or dishonour of Cheque will attract a penalty of Rs 500/- per cheque. Further delay in payment will also attract a fine of Rs 10/- per day from the date of commitment to the date of payment received.
